The Omaha Pathways Community HUB is dedicated to improving health outcomes by connecting individuals and families to the care and resources they need. Since launching services in 2024, we’ve seen remarkable progress—growing a dedicated team of Community Health Workers, strengthening partnerships, and reaching dozens of families with life-changing support.

As we continue expanding services and deepening connections in the community, our goal remains the same: to remove barriers, improve health equity, and ensure every pregnant person has the support they need for a healthy future.

By the Numbers

Our work has seen tremendous growth and impact so far, and the data below highlights some of these accomplishments.

  • 84 pregnant people served, meaning at least one need was addressed.
  • 322 face-to-face visits where participants discussed needs, accessed resources, and set goals.
  • 264 health education lessons completed, showing participants’ commitment to health and wellness.
  • 257 Pathways completed across 12 need areas, with outcomes achieved.

Pathways help measure progress toward addressing a social risk factor. Each Pathway has a defined outcome for success. For example, someone is safely and stably housed for 30 days.

Impact Stories

» Motherhood reimagined. Joining our CHW program at 18 weeks pregnant, one participant successfully balanced her first semester in interior design school while working full time. The support from CHW Artnetta and referrals to financial resources during postpartum have allowed her to thrive at work and home. She credits the CHW’s support for helping prevent postpartum depression, a challenge she faced during a previous pregnancy.

» Navigating a challenging pregnancy. Joining the program late in her pregnancy, one participant faced significant complications requiring close medical oversight and numerous appointments. With the father of the baby providing support and the guidance of her CHW, she managed to follow the medical guidelines and gradually built trust with her healthcare providers. She found comfort in her support network and eventually had a health delivery.  As a new mother, she continues to receive support from her CHW to find stable housing and assist the father of the baby in finding employment.
