The Omaha Pathways Community HUB is dedicated to improving health outcomes by connecting individuals and families to the care and resources they need. Since launching services in 2024, we’ve seen remarkable progress—growing a dedicated team of Community Health Workers, strengthening partnerships, and reaching dozens of families with life-changing support.

As we continue expanding services and deepening connections in the community, our goal remains the same: to remove barriers, improve health equity, and ensure every pregnant person has the support they need for a healthy future.

This page serves as a place to track our progress, share impact stories, and celebrate the achievements of our participants and partners. Check back for quarterly updates or sign up for our quarterly emails. (Page last updated February 2026.)

Impact Stories

» Building a new path. When one participant enrolled, she was navigating housing instability, unemployment, and limited resources. With her CHW, she prioritized long-term goals, completing the FAFSA and preparing to start school. Together, they secured essential supplies, including a phone, and connected her with counseling support. She has remained sober, completed her probation period, and is moving forward with renewed stability and purpose.

» Surrounded by support. One participant enrolled in Pathways through another care coordination agency and was referred to A Mother’s Love for additional support. Through strong partnerships, she received needed supplies and was connected to IBBG for doula services. This coordinated, wraparound approach ensured she was supported at every step, demonstrating how collaboration across organizations strengthens care for families.
