Progress
The Omaha Pathways Community HUB is dedicated to improving health outcomes by connecting individuals and families to the care and resources they need. Since launching services in 2024, we’ve seen remarkable progress—growing a dedicated team of Community Health Workers, strengthening partnerships, and reaching dozens of families with life-changing support.
As we continue expanding services and deepening connections in the community, our goal remains the same: to remove barriers, improve health equity, and ensure every pregnant person has the support they need for a healthy future.

By the Numbers
Our work has seen tremendous growth and impact so far, and the data below highlights some of these accomplishments.
- 621 face-to-face visits where participants discussed needs, accessed resources, and set goals.
- 445 health education lessons completed, showing participants’ commitment to health and wellness.
- 436 Pathways completed across 15 areas of need with outcomes achieved.
Pathways help measure progress toward addressing a social risk factor. Each Pathway has a defined outcome for success. For example, someone is safely and stably housed for 30 days.

Impact Stories
» A network of care. During a visit, one CHW met a participant’s grandmother, who quickly understood and appreciated the value of the Pathways program. Inspired, she shared the CHW’s contact information with a friend—who then shared it with her daughter—who then passed it along to her niece. Each connection led to someone enrolling in the program. It’s a reminder that when CHWs are embedded in the community, support spreads organically through conversations, care, and connection.
» Building confidence. One participant recently worked with her CHW to apply for utility assistance and submit multiple applications for financial support. With each step, her stress began to ease. Together, they built a plan to stabilize her finances and help her feel more in control of her future. Her CHW’s support—and belief in her—helped turn overwhelm into self-trust.
