Is the Omaha Pathways Community HUB right for me?

The Omaha Pathways Community HUB brings together a network of Community Health Workers to help you navigate life’s challenges. They will help you access, manage, and plan care activities that lead to better health outcomes for you and your family. The HUB’s Pathways are a step-by-step process to address your needs and help you achieve your health and wellness goals.

WHAT IS THE OMAHA PATHWAYS COMMUNITY HUB?

The HUB is a group of community organizations, health care providers, and others working together to make sure everyone in our community can reach their health goals.

WHAT IS A COMMUNITY HEALTH WORKER?

A Community Health Worker (CHW) is a certified health professional who is trained to connect you with social, medical and behavioral health resources. They will partner with you to help you achieve your health and wellness goals.

HOW MUCH DOES IT COST?

Pathways HUB services are FREE to participants.

WHAT SHOULD I EXPECT?

You will be partnered with a Community Health Worker (CHW) from A Mother’s Love, Charles Drew Health Center or CHI Health. You will meet in person with your CHW at least once a month to discuss areas where you need support, work on your care plan, and update your health goals.

WHAT IS MY ROLE?

First, you will work with your Community Health Worker to pick priorities to work on together. Then, participate in the process and take action to make healthy changes.

WHO CAN JOIN THE PROGRAM?

The HUB currently serves people in Douglas or Sarpy Counties. You must be pregnant to enroll in the program, and you can continue to receive services for up to a year after delivery.

IS THE PATHWAYS HUB PROGRAM RIGHT FOR ME?

If you had a previous pregnancy or birth that faced challenges, the HUB might be right for you. The HUB also may be a good fit if you are pregnant and need assistance with these types of resources:

• Housing or utilities
• Food assistance
• Safety
• Transportation
• Education
• Baby supplies
• Child care
