Free Care Coordination
Do you need pregnancy support, childcare, or baby supplies? Do you need assistance getting food and medicine? Health insurance? A primary-care doctor? Safe and stable housing? Job training and education? The Omaha Pathways Community HUB can help with these needs and more.
The Omaha Community Pathways HUB brings together a network Community Health Workers to help you navigate life’s challenges. They will help you access, manage, and plan care activities that lead to better health outcomes for you and your family. The HUB’s Pathways are a step-by-step process to address your needs and help you achieve your health and wellbeing goals.
It is the Omaha Pathways Community HUB’s mission to help guide our communities to better health. All services are provided free of charge.

How to Enroll
Currently, the HUB is serving pregnant women and individuals living in Douglas and Sarpy Counties.
Individuals, community partners, or health care providers can make referrals to the HUB. To refer a new participant to the HUB please complete the following online form and someone will reach out within 2 business days. (Print/Email form option)
