The Omaha Community Foundation is excited to announce a Request for Applications (RFA) for community-based nonprofit organizations interested in partnering with our Omaha Pathways Community HUB (HUB) to improve birth outcomes in Douglas and Sarpy Counties in Nebraska.
The Omaha Pathways Community HUB is a demonstration project to implement the Pathways Community HUB Institute® Model. This pilot is focused on improving birth outcomes by reducing the incidence of low birth weight and/or preterm babies in Omaha with a priority to serve Black pregnant women and individuals. The project is currently funded by CHI Health, Healthy Blue Nebraska, Nebraska Total Care, and United Healthcare.
The RFA is seeking applications from community-based nonprofit organizations that have community health workers (CHWs) on staff or are willing to hire and supervise CHWs to be trained and deployed to provide community-based care coordination through the evidence-based PCHI® Model. Organizations with a successful history in care coordination, community outreach, home-based services, and/or CHW programming (or a similar type of worker) are encouraged to consider this opportunity.

Selected applicants will be considered Care Coordination Agencies (CCAs) of the HUB. The selected agencies will be eligible for start-up funding, engagement fees, and outcome-based payments for successfully engaging the focus population in implementing the PCHI® Model of assessing and addressing access to social and medical resources. CCAs will be partners with the HUB in the implementation of this evidence-based model to ensure we realize the intended impacts and outcomes for our community.
